The Role As a Service Coordinator, you’ll plan, coordinate, and monitor services under the Support at Home 2025 program & NDIS . This role ensures clients receive timely, quality, and individualised support that aligns with their goals and wellbeing. Key Responsibilities Conduct client assessments and develop personalised Care PlansCoordinate and monitor services within approved budgetsLiaise with clients, carers, and allied health professionalsMaintain accurate client records and documentationSupport and guide community care staffSupport at Home quality standards & NDIS standards. About You You’re an organised, empathetic professional who enjoys building relationships and delivering quality client outcomes. Essential Certificate IV in Ageing, Disability or Community ServicesCurrent driver’s licence and reliable vehicleCurrent First Aid & CPR CertificateNDISWSCWWCCExperience in service coordination or case managementExcellent communication and computer skills Desirable Experience with My Aged Care and aged care compliance systemsExperience with NDIS and compliance systems Why Join Subee?
