Support Coordinator (NDIS) – Temporary Maternity Leave Contract | Full Time

Join our team at TKCC and make a meaningful impact for NDIS participants.
We are seeking a compassionate Support Coordinator to join us on a full-time temporary maternity leave contract.

About the Role

As a Support Coordinator, you will be:

  • Working collaboratively with participants to implement their NDIS plans and achieve their goals.

  • Coordinating, engaging and monitoring service providers and community supports.

  • Supporting participants to build confidence to make informed decisions.

  • Providing advice on the NDIS environment, funding, and service options.

  • Completing all required administrative duties, including reporting, budget tracking, service bookings, and case documentation.

  • Maintaining working relationships with internal team members, the Office of Public Guardian, Public Trustee, NDIA, and local service providers.

  • Contributing to continuous improvement, business goals, and quality service delivery.

What We’re Looking For

To be successful, you will have:

  • A tertiary qualification in Community Services, Mental Health, Disability, or related experience.

  • Minimum 2 years’ experience in Support Coordination, disability, or mental health services.

  • Strong understanding of NDIS legislation, Quality & Safeguards, reportable incidents, and the NDIS Price Guide.

  • Demonstrated case management experience, including planning, implementing, and reviewing service plans.

  • Ability to work autonomously, meet targets, manage competing priorities, and problem-solve complex issues.

  • Excellent relationship-building skills with participants, providers, and stakeholders.

  • Proficiency in MS Office, NDIS systems (PRODA, MyPlace), and client management software.

  • A current Ochre Card (WWCC), NDIS Worker Screening Check, Police Check, First Aid Certificate, COVID-19 vaccination evidence, NT Driver Licence, and reliable vehicle.
